Taylor Swift didn’t need to perform at the 2026 VMAs to steal the spotlight. The pop superstar premiered the music video for “Patient Zero” during the ceremony, celebrating the release alongside Dakota Johnson , who star in the highly anticipated project. Swift also made history by receiving the inaugural Artist Director award , becoming the most awarded artist in Video Music Awards history .

The song is part of “The Life of a Showgirl: The Encore” , the extended edition of her latest album, which arrived this Friday, September 25 , featuring four additional tracks. Swift surprised fans by announcing the project earlier this week.

Everything we know about the video

The music video was directed by Taylor Swift herself, while cinematography was handled by Emmanuel Lubezki , the three-time Oscar winner for “Gravity,” “Birdman,” and “The Revenant.”

The project features two major Hollywood stars as its protagonists: Dakota Johnson and Colin Farrell .

On Thursday, during her appearance on CBS Mornings , Swift shared a 13-second teaser, a number that holds special meaning within her universe of clues and references. She later posted the preview on social media with only two words:

The footage shows Swift turning on a faucet inside a dark bathroom, a scene where she and Johnson face each other at a sink as if they were reflections of one another, both women jumping into a pool, a cemetery featuring a coffin and a black rose, Farrell stepping out of a car, and all three protagonists dressed entirely in black.

The teaser also shows a close-up of Swift crying, where fans quickly noticed her engagement ring. Everything is accompanied by a soundtrack featuring Irish-inspired strings and ends with what appears to be the title of a movie.

So far, Swift has not revealed the plot or the characters. As often happens with her projects, fans have spent days trying to decode every image and connect the clues to previous references. The answer will arrive during the ceremony.

When and where to watch the premiere

The 2026 VMAs will begin at 7:30 PM ET (4:30 PM PT) and will air on CBS , with a simultaneous broadcast on MTV and streaming through Paramount+ Premium .

CBS and MTV confirmed that the “Patient Zero” music video will have its world premiere during the ceremony, although the exact moment has not yet been revealed. Everything suggests it could coincide with the presentation of the Artist Director award.

Until now, Taylor Swift and Beyoncé shared the record for the most awards in VMAs history, with 30 trophies each .

With the new Artist Director honor , Swift will reach 31 awards and become the most decorated artist in the history of the event.

She also enters the ceremony with 11 competitive nominations, including Video of the Year for “The Fate of Ophelia” and Best Direction for “Opalite.” According to record analyses, she could finish the night with up to 42 total awards.

She also has the chance to become the first artist to win Artist of the Year three times.

Anyone hoping to watch the premiere of “Patient Zero” as soon as possible will need to tune in from the beginning of the ceremony. The video will later be available on YouTube, but its world premiere will take place exclusively during the VMAs.
