Hayden Panettiere ’s death has taken another painful turn after the Greenville County Coroner’s Office finally confirmed what caused the 36-year-old actress to die on August 16 .

The Heroes star was found unresponsive at a South Carolina residence on, with emergency crews unable to revive her despite attempting advanced life-saving measures.

The coroner ruled that Panettiere died from the toxic effects of fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine. Her death was classified as accidental.

“The cause of death has been ruled as toxic effects of fentanyl, 4-ANPP, Alprazolam, Methocarbamol and Quetiapine,” the coroner’s official statement said. “The manner of death has been ruled accident.”

The findings provide the first official explanation for the actress’s death after weeks of uncertainty. Authorities had previously confirmed that no trauma was discovered during her autopsy.

Although she became internationally known through Heroes , Panettiere began acting as a child as she later starred in Nashville , Scream 4 and Scream VI , cementing a career that spanned decades.

Her death has since prompted tributes from friends, family, former co-stars and fans, with a memorial held in Malibu earlier this month. The coroner’s ruling now provides an official answer to the question surrounding her final moments.

What happened to Hayden Panettiere?

A 911 call reporting cardiac arrest was received at approximately 1:51 p.m. on August 16, where emergency personnel arrived and found Panettiere unresponsive inside the residence.

EMS crews began advanced cardiac life support measures, but their efforts were unsuccessful. She was pronounced dead at 2:32 p.m.

Officials also determined that there were no traumatic injuries that contributed to her death. Police had previously said there was no indication of foul play surrounding the actress’s passing.

The substances identified by officials include fentanyl, a powerful synthetic opioid, along with 4-ANPP, which can be used as a chemical intermediate in illicit fentanyl production.

Alprazolam, better known by the brand name Xanax, is a prescription medication used for anxiety and panic disorders. Methocarbamol is a muscle relaxant, while quetiapine is an antipsychotic medication.

The revelation comes after years in which Panettiere openly discussed her struggles with alcohol and opioids, including the devastating effect addiction had on her personal life and career.

In 2022, she described her addiction as having spiraled “out of control” and said she had reached a point where she “couldn’t live without” alcohol and opioids.

Eventually, her former fiancee Wladimir Klitschko decided he could no longer be around her and the battle against substance abuse also led to their daughter moving to Ukraine after Panettiere lost custody.
